Change "\renewcommand{\indexsubitem}{(...)}" to "\setindexsubitem{(...)}"
everywhere.

Added quote_plus() and unquote_plus(), to do space/plus substitutions
...
for form field values.
Also corrected substitution example (the original changed the spelling
of "conolly" to "connolly" :).
