make 2to3 use unicode internally on 2.x
This started out as a fix for #2660, but became this large refactoring
when I realized the dire state this was in. 2to3 now uses
tokenize.detect_encoding to decode the files correctly into unicode.

Added a fixer that cleans up a tuple argument to isinstance after the tokens
in it were fixed. This is mainly used to remove double occurrences of

This extends fix_xrange to know about the (mostly) same special contexts as fix_dict (where a special context is something that is guaranteed to fully consume the iterable), adding list() calls where appropriate. It also special-cases "x in range(y)".
