bpo-31370: Remove support for threads-less builds (#3385)

* Remove Setup.config
* Always define WITH_THREAD for compatibility.

This option is not really useful anymore in the 21st century. Removing lots
of conditional paths allows us to simplify the code base, including in
difficult to maintain low-level internal code.
