Made it real. Changed locking() to work with file descriptors instead

of Python file objects.  Added open_osfhandle() (Mark had done some
work for that), get_osfhandle(), setmode(), and the console I/O
functions kbhit(), getch(), getche(), ungetch(), and putch().
