PyUnicode_EncodeFS() raises an exception if _Py_wchar2char() fails

* Add error_pos optional argument to _Py_wchar2char()
 * PyUnicode_EncodeFS() raises a UnicodeEncodeError or MemoryError if
   _Py_wchar2char() fails
