We're no longer using _Py_IDENTIFIER() (or _Py_static_string()) in any core CPython code. It is still used in a number of non-builtin stdlib modules.
The replacement is: PyUnicodeObject (not pointer) fields under _PyRuntimeState, statically initialized as part of _PyRuntime. A new _Py_GET_GLOBAL_IDENTIFIER() macro facilitates lookup of the fields (along with _Py_GET_GLOBAL_STRING() for non-identifier strings).

The core of the change is in:
* (new) Include/internal/pycore_global_strings.h - the declarations for the global strings, along with the macros
* Include/internal/pycore_runtime_init.h - added the static initializers for the global strings
* Include/internal/pycore_global_objects.h - where the struct in pycore_global_strings.h is hooked into _PyRuntimeState
* Tools/scripts/generate_global_objects.py - added generation of the global string declarations and static initializers
I've also added a --check flag to generate_global_objects.py (along with make check-global-objects) to check for unused global strings. That check is added to the PR CI config.
The remainder of this change updates the core code to use _Py_GET_GLOBAL_IDENTIFIER() instead of _Py_IDENTIFIER() and the related _Py*Id functions (likewise for _Py_GET_GLOBAL_STRING() instead of _Py_static_string()). This includes adding a few functions where there wasn't already an alternative to _Py*Id(), replacing the _Py_Identifier * parameter with PyObject *.
The following are not changed (yet):
* stop using _Py_IDENTIFIER() in the stdlib modules
* (maybe) get rid of _Py_IDENTIFIER(), etc. entirely -- this may not be doable as at least one package on PyPI using this (private) API
* (maybe) intern the strings during runtime init

Move almost all private functions of Include/cpython/fileutils.h to
the internal C API Include/internal/pycore_fileutils.h.
Only keep _Py_fopen_obj() in Include/cpython/fileutils.h, since it's
used by _testcapi which must not use the internal C API.
Move EncodeLocaleEx() and DecodeLocaleEx() functions from _testcapi
to _testinternalcapi, since the C API moved to the internal C API.

Correctly hand various failure modes when resizing an mmap on Windows:
* Resizing a pagefile-backed mmap now creates a new mmap and copies data
* Attempting to resize when another mapping is held on the same file raises an OSError
* Attempting to resize a nametagged mmap raises an OSError if another mapping is held with the same nametag
* Make functools types immutable
* Multibyte codec types are now immutable
* pyexpat.xmlparser is now immutable
* array.arrayiterator is now immutable
* _thread types are now immutable
* _csv types are now immutable
* _queue.SimpleQueue is now immutable
* mmap.mmap is now immutable
* unicodedata.UCD is now immutable
* sqlite3 types are now immutable
* _lsprof.Profiler is now immutable
* _overlapped.Overlapped is now immutable
* _operator types are now immutable
* winapi__overlapped.Overlapped is now immutable
* _lzma types are now immutable
* _bz2 types are now immutable
* _dbm.dbm and _gdbm.gdbm are now immutable
This works by not caching the handle and instead getting the handle from
the file descriptor each time, so that if the actual handle changes by
fd redirection closing/opening the console handle beneath our feet, we
will keep working correctly.
The mmap module set MAP_SHARED flag when map anonymous memory, however VxWorks
only support MAP_PRIVATE when map anonymous memory, this commit clear MAP_SHARED
and set MAP_PRIVATE.
The final addition (cur += step) may overflow, so use size_t for "cur".
"cur" is always positive (even for negative steps), so it is safe to use
size_t here.

fstat may block for long time if the file descriptor is on a
non-responsive NFS server, hanging all threads. Most fstat() calls are
handled by _Py_fstat(), releasing the GIL internally, but but
_Py_fstat_noraise() does not release the GIL, and most calls release the
GIL explicitly around it.
This patch fixes last 2 calls to _Py_fstat_no_raise(), avoiding hangs
when calling:
- mmap.mmap()
- os.urandom()
- random.seed()
