Neal Norwitz 7728b4719e SF #994605, fcntl example is broken 
The last call to fcntl (which wasn't changed) doesn't work for me, but
the first part works now.

Backport candidate.

Michael W. Hudson f008998668 Fix bug 
[ 555817 ] Flawed fcntl.ioctl implementation.

with my patch that allows for an array to be mutated when passed
as the buffer argument to ioctl() (details complicated by
backwards compatibility considerations -- read the docs!).

Fred Drake 6a6bb1828b Clarify that on some systems, lockf() using LOCK_EX requires that the file 
is opened for writing; this closes SF bug #485342.

Added notes that file objects are also accepted in the place of file
descriptors.

Fred Drake c37b65ee10 Clean up some markup cruft. A number of the macros that take no 
parameters (like \UNIX) are commonly entered using an empty group to
separate the markup from a following inter-word space; this is not
needed when the next character is punctuation, or the markup is the
last thing in the enclosing group.  These cases were marked
inconsistently; the empty group is now *only* used when needed.

Guido van Rossum cf6905f986 Clarify the example by explicitly importing the fcntl module -- this 
avoid being fooled into thinking that fcntl and FCNTL are the same
thing -- they aren't!  (fcntl is the extension, FCNTL.py is h2py
output that defines all the constants).

(XXX The example is still weird -- I think there's a more portable way
to do locking now.  That's for someone else to fix...)

Change "\renewcommand{\indexsubitem}{(...)}" to "\setindexsubitem{(...)}"
everywhere.
